Change of Address and Name

To ensure that you receive grade reports and other information, it is important to keep Student Services informed of any change in your contact information. Student Services processes all changes of name, address, and telephone number.

Change of Address
Please submit the Change of Address Form to Student Services to record any changes in your address and/or telephone number in all departments. Address changes also may be submitted under the My Profile section of Cougar Web.

Change of Name
You may record a name change by submitting the Change of Name Form to Student Services. All name change requests must be accompanied by official documentation, which may include a copy of a marriage certificate or a divorce decree. Requests for name change will not be processed without sufficient documentation.
